Veteran/Service Dog Program

Fairview Animal Hospital will provide free preventive care for up to five, veteran-owned, PTSD service dogs, for the life of the pet, as long as it is owned by the veteran.* (We will include the same services as in our Preventive Care Plans.) If you are a veteran with a PTSD service dog and want to be considered for the program, please contact Stacey McKnight, our Practice Manager, at 770-389-9757.

If you are entered into the program you will need to complete a Client Information form and a Patient Information form. You must bring your pet in for services no later than 30 days after their due date. (We will remind you via text, email, postcard and/or telephone, as you prefer.)  Failure to bring your dog in for services within 30 days after the due date will result in removal from the program.  If you are removed from the program, you may reapply and will be considered if an opening is available.  If there are no openings, you will be placed on a waiting list.
